Activity Description

Tubal sterilization is the intentional occlusion or partial or complete removal of the fallopian tubes to provide permanent contraception in females, and it is the most common method of contraception used worldwide. The procedure is indicated when it is desired by the patient for permanent contraception; the only absolute contraindication is a lack of informed consent from the patient. Therefore, the consent process should stress the permanent nature of the procedure and review the entire spectrum of alternative contraceptive options with a focus on long-acting reversible contraceptives (LARCs), including the intrauterine device (IUD) and contraceptive implant, which both have efficacy rates similar to traditional tubal sterilization techniques. 

In recent years, complete bilateral salpingectomy has become the sterilization procedure of choice because it appears to decrease the risk of future epithelial ovarian cancer and post-sterilization contraceptive failure compared with traditional sterilization techniques without increasing surgical risk. Previously, interval sterilization was often accomplished by laparoscopically occluding the tubes with clips, bands, or electrocautery. In contrast, postpartum sterilization was typically performed as a partial salpingectomy through a mini-laparotomy incision. These methods are still viable options for sterilization and may be preferable in some circumstances.

This activity for healthcare professionals is designed to enhance the learner’s competence in recognizing the indications and contraindications for tubal sterilization; understanding the ethical concerns regarding sterilization in special populations, including incarcerated individuals; performing appropriate preoperative counseling, which involves a discussion of the risks, benefits, and complications of tubal sterilization, including the risk of regret; explaining the techniques available to perform tubal sterilization; and understanding the role of the interprofessional team in caring for patients who undergo the procedure.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Identify the indications and contraindications for tubal sterilization, and know how to effectively counsel patients regarding the procedure.

  • Implement the proper techniques for performing tubal sterilization.

  • Identify the complications of tubal sterilization.

  • Employ interprofessional team strategies for optimizing care coordination and communication to optimize patient outcomes after tubal sterilization.
