Activity Description

Obstetric perineal lacerations, frequently occurring during childbirth, can affect the perineum, labia, vagina, and cervix. While most lacerations heal without long-term issues, severe cases can result in prolonged pain, sexual dysfunction, and embarrassment. These severe lacerations need prompt identification and proper repair during delivery. Perineal trauma is common in vaginal births, with lacerations occurring spontaneously or due to medical interventions like episiotomies. Most of these lacerations are first- or second-degree and usually require suturing. However, severe lacerations, particularly obstetric anal sphincter injuries, can lead to significant complications like pelvic floor dysfunction and incontinence. Risk factors for perineal lacerations include first-time childbirth, operative deliveries, larger babies, certain fetal positions, and specific maternal factors such as age and ethnicity. Episiotomies, particularly the midline type, are linked with higher rates of severe lacerations, although mediolateral episiotomies, while more complicated to repair, can reduce severe tears. Follow-up care following perineal laceration repair is crucial and includes pain management, preventing constipation, and monitoring for urinary retention. Preventative measures, including perineal massage in late pregnancy and careful intrapartum techniques, can help reduce the risk and severity of perineal lacerations. 

Participants in this course gain comprehensive knowledge about obstetric perineal lacerations, including risk factors, preventive measures, and best practices for repair and follow-up care. The course emphasizes the importance of early identification and effective management to prevent long-term complications. Clinicians learn to recognize various degrees of lacerations, select and perform appropriate repair techniques, and implement follow-up care to ensure optimal healing.

This activity has been designed to meet the educational needs of physicians, physician associates, nurses, pharmacists, and nurse practitioners.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Identify the risk factors for obstetric perineal lacerations, including maternal, fetal, and delivery-related factors.

  • Differentiate between the degrees of perineal lacerations to ensure accurate diagnosis and appropriate treatment.

  • Apply evidence-based techniques for suturing and repairing various degrees of perineal lacerations.

  • Collaborate with an interprofessional team, including obstetricians, midwives, nurses, and pelvic floor therapists, to provide comprehensive care for patients with perineal lacerations.
