Activity Description

Hemolytic transfusion reactions are a possible complication of blood transfusions. Hemolysis is the rupture of red blood cells which can occur intravascularly or extravascularly. These reactions can be immune-mediated or non–immune-mediated. Immune hemolytic transfusion reactions occur due to mismatch or incompatibility of the recipient with the donor products and are divided into acute versus delayed reactions. This activity illustrates the pathophysiology, evaluation, and management of hemolytic transfusion reactions and highlights the role of the interprofessional team in the prevention of these reactions.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Identify potential risk factors for hemolytic transfusion reactions, including patient history, previous transfusion reactions, and incompatible blood product administration.

  • Differentiate between immune-mediated and non–immune-mediated hemolysis in transfusion reactions, considering the timing of onset and clinical manifestations.

  • Assess and treat patients for signs and symptoms of acute hemolytic transfusion reactions within 24 hours posttransfusion, and for delayed reactions up to 30 days posttransfusion. Identify the importance of recognizing delayed post-transfusion antibodies for possible future transfusions.

  • Coordinate efforts to review and update institutional transfusion protocols and training to enhance patient safety and minimize the occurrence of preventable hemolytic transfusion reactions.
