Activity Description

Bacterial endophthalmitis is an intraocular infection that can have devastating consequences, leading to decreased vision and irreversible blindness if not promptly recognized. The sterile environment of the eye can be breached by penetrating trauma, surgery, endogenous sources via bacteria in the patient's circulatory system, or fungi invading and multiplying within the vitreous or aqueous humor. Notably, bacteremia or fungemia might be transient, leading to presentations devoid of systemic infection symptoms. Advanced molecular diagnostic methodologies, currently employed in research settings, facilitate expedited pathogen identification when conventional culture methods yield negative results. Therapeutically, the intravitreal administration of antibiotics constitutes the cornerstone of management, supplemented by surgical intervention in select scenarios. The causative pathogen influences the prognostic trajectory; for instance, endophthalmitis induced by coagulase-negative staphylococci typically portends a more favorable outcome than streptococcal-origin endophthalmitis. Given its potential for rapid progression and severe visual sequelae, endophthalmitis is categorized as an ophthalmologic emergency, necessitating decisive diagnostic and therapeutic interventions to preserve visual function. This course outlines and describes the evaluation and management of bacterial endophthalmitis and reviews the role of the interprofessional team in collaborating to provide coordinated care to enhance patient outcomes.

  • Identify the etiology of bacterial endophthalmitis, differentiating between mild and severe pathogens.

  • Select the appropriate history, physical, and evaluation of bacterial endophthalmitis.

  • Compare the management options available for bacterial endophthalmitis.

  • Implement interprofessional team strategies for improving care coordination and communication to mitigate long-term sequelae of bacterial endophthalmitis.
