Activity Description

Aortic valve replacement is the gold standard treatment for patients with severe or symptomatic aortic insufficiency or aortic stenosis. The standard method of replacing the aortic valve has been via a median sternotomy approach, but minimally invasive aortic valve surgery has evolved as an alternative, offering improved outcomes. Traditional sternotomy, although practical, may lead to inadequate healing, heightened pain, and prolonged recovery, especially in patients with conditions like osteoporosis or diabetes. Minimally invasive techniques offer a solution involving various access routes to the aortic valve, such as right anterior minithoracotomy, ministernotomy, or percutaneous methods. These procedures, involving smaller incisions and less postoperative pain, reduce intensive care unit stays and overall hospital length of stay. They also showcase potential benefits like enhanced cosmetic outcomes and faster recovery. Though more costly, these evolving approaches signify advancements in aortic valve replacement, promoting tailored, patient-centered care.

Aortic valve replacement from standard sternotomy to minimally invasive aortic valve replacement surgery, encompassing approaches like right anterior minithoracotomy, ministernotomy, and percutaneous methods are discussed. Indications for minimally invasive aortic valve replacement surgery, including severe or symptomatic aortic insufficiency or stenosis, are elucidated alongside contraindications. The course emphasizes the interprofessional team's role in treating patients with aortic valve disease, promoting collaborative decision-making and tailored care. By engaging in this course, healthcare professionals comprehensively understand minimally invasive aortic valve replacement surgery, fostering competence in patient evaluation, selection, and perioperative management.

Target Audience

This activity has been designed to meet the educational needs of physicians, physician associates, nurses, pharmacists, and nurse practitioners.

Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Identify suitable candidates for minimally invasive aortic valve surgery based on individual patient factors, such as age, comorbidities, and valve pathology.

  • Screen patients thoroughly, considering factors like chest wall deformities, radiation therapy history, and prior chest surgeries, to determine eligibility for minimally invasive procedures.

  • Differentiate between various minimally invasive approaches and open heart surgery, considering the pros and cons of each method.

  • Collaborate with other healthcare professionals, such as intensivists and nurses, to provide comprehensive perioperative care and postoperative monitoring for patients undergoing minimally invasive procedures.
