Activity Description

Rib fractures are a common injury found in trauma patients presenting to the Emergency Department. Rib fractures are associated with significant morbidity and mortality, especially in older adults. Pulmonary complications like pneumonia can develop days after the injury due to compromised respiratory function secondary to breathing-related pain. Therefore, adequate analgesia is an essential component of rib fracture management and has been shown to reduce morbidity and mortality by allowing improved respiratory status. Intravenous opioids historically have been a mainstay of treatment but have many potential side effects, including respiratory depression, depressed cough reflex, and delirium in at-risk populations. The ultrasound-guided serratus anterior plane block (SAPB) is a popular alternative to intravenous pain management due to its efficacy, relative ease, and limited side-effect profile for multiple rib fractures. This activity reviews the indications, contraindications, and technique for performing an ultrasound-guided serratus anterior plane block. It highlights the role of the interprofessional team in managing the analgesia of patients with acute rib fractures.
