Activity Description

A pulmonary arteriovenous malformation (PAVM) is a rare pulmonary condition characterized by a pathological right-to-left shunt that disrupts normal gas exchange and venous blood filtration in the pulmonary circulation. Often associated with hereditary hemorrhagic telangiectasia (HHT), PAVMs can lead to severe complications such as ischemic stroke, myocardial infarctions, cerebral abscesses, massive hemoptysis, and hemothorax.

Clinicians participating in this activity on PAVM can expect to gain a deep understanding of the pathophysiology of PAVMs, their association with HHT, and the potential severe complications. This activity reviews the signs and symptoms associated with PAVMs, equipping clinicians to consider this condition in their differential diagnosis, particularly for patients with epistaxis, hypoxemia, and exertional dyspnea. Learning Objectives

At the conclusion of this activity, the learner will be better able to:

  • Identify patients at risk for pulmonary arteriovenous malformation (PAVM) by recognizing symptoms such as hypoxemia, dyspnea, and exercise intolerance.

  • Screen individuals with suspected PAVM efficiently using appropriate diagnostic methods, including contrast-enhanced CT, pulmonary angiography, and transthoracic saline bubble contrast echocardiography.

  • Implement timely and appropriate interventions for confirmed PAVM cases, considering factors such as feeding artery diameter and patient symptoms.

  • Coordinate care for patients with PAVM, including long-term follow-up plans, to optimize outcomes and prevent complications.
